Medical Forms for Trip

    SO TRIP TO WASHNIGTON, DC


     


    I am very happy to be able to go on the DC trip with the SO this year!  I would like to take this opportunity to get things in place so that we all have a safe, healthy trip!


     


    1. Please remember that this is a school trip!  All laws that pertain to medication delivery while in school are in effect on this trip!

    2. If your child already has a medication order in my office, I have everything that I need, and will be bringing this medication with me. 

    3. If you would like your child to have access to medicine (prescription or over the counter medicine) on the trip that isn’t already ordered (inhalers, allergy medicine, motrin, motion sickness medicine, eye drops etc.) you MUST obtain and submit a doctor’s order AND bring in the medicine to me in its original,  labeled container by Tuesday, May 30th.  Expired medicine and medicine in unlabeled containers CAN NOT BE ACCEPTED.   A copy of the medication form for your doctor to sign is attached for your convenience.

    4. Please do not give your child medication to carry unless your doctor has an order in with me that allows them to do so.  A form is attached that your doctor can sign that will allow your child to carry and self-administer medication.  If you would like your child to be able to carry and self-administer any medication (prescription or over the counter), the doctor’s note should read that (your child’s name) “may carry and self-administer” (their medication name).  This note MUST be signed by the physician.

    5. Morning medications will be delivered to students in the hotel at breakfast.  Bedtime medicines will be delivered after arriving back to the hotel for the night.  I will carry your child’s “as needed” medications such as asthma inhalers, Motrin, Tylenol etc. and if your child requests it, will deliver it to them as ordered.  Please refer to the itinerary for a time frame reference for morning and bedtime medication delivery times.

    6. If your child has health needs or concerns that you would like to discuss with me, please call me prior to June 1st  at (914) 257-5790.  Individual questions and concerns should not be left until the day of the trip as the possibility that I will be able to address your concerns satisfactorily at that time is unlikely.
